Please ask for clarification on any portion that you do not understand. Please initial after each statement indicating that you understand and agree to the statement

I understand that Bindee Yoga LLC incorporates movement, and guided experiences. I understand that there is always an inherent risk when participating in physical activities. I agree to let the therapist(s) or Bindee Yoga LLC representatives know of any physical limitations I might have, or any physical activities I do not wish to participate in.
I understand the training program is not intended to substitute medical and licensed mental health services.
I hereby release Bindee Yoga LLC and any Representatives of Bindee Yoga LLC from responsibility for any injuries I may sustain as a result of participation in this program.
While cleaning of the space and equipment is done regularly, I understand that participating in Bindee Yoga LLC classes involves potential risks, including but not limited to the potential risk of exposure to communicable diseases, including but not limited to any strain or variation of viruses or infections, which may result in personal injury, or illness.
